Personal Information Collection Statement

Provision of personal data is voluntary, but failure to supply such data may result in the Netball Hong Kong, China Limited (“NHKC”) being unable to process your / the Player’s registration for the Program or to provide facilities and services to you / the Player. I / the Player hereby acknowledges and consent to the collection, use, and processing of Personal Data by NHKC in accordance with the Personal Data (Privacy) Ordinance (Cap. 486) (“PDPO“) of Hong Kong. “Personal Data” means “personal data” as defined in the PDPO collected from you / the Player by NHKC in relation to you / the Player.

Purposes of Collection: The Personal Data collected will be used for (i) registration in the Program, (ii) carrying out NHKC’s operations and provision of facilities and services (including managing the Program), (iii) offering, providing and marketing the facilities and services offered by NHKC, or of any of NHKC’s sponsors, (iv) communicating with you, (v) providing alerts, updates, materials or information about NHKC’s facilities and services, (vi) complying with the laws of any applicable jurisdiction and with any internal policies of NHKC, and (vii) any other purpose relevant to the Program.

Types of Personal Data Collected: The Personal Data that may be collected from you / the Player includes, but is not limited to (i) contact information such as your name, email address, mailing address, telephone number, and fax number, (ii) unique identifiers such as your HKID and passport number, (iii) demographic information such as age, gender, nationality, marital status, preferences, education background, and employment history, (iv) medical information, (v) any other information relevant to the Program, and (vi) any of the above Personal Data in relation to your / the Player’s parents and/or child.

Disclosure and Transfer of Personal Data: The Personal Data collected will be used solely for the purposes stated at paragraph 1 above and will be kept confidential. However, NHKC may disclose the Personal Data to third parties, including but not limited to event sponsors, service providers, medical personnel, or government authorities, only when necessary and in compliance with the PDPO.

Data Retention: NHKC will retain the Personal Data for as long as is necessary to fullfill the purposes for which it was collected, or as required by applicable laws and regulations.

Access and Correction: You / the Player have the right to request access to and correction of the Personal Data held by NHKC by making a request to [email protected]

Other: I / the Player agree to be photographed and filmed for NHKC to use for the purpose of promoting and marketing netball in Hong Kong.

In consideration of and as a condition of the above listed player’s (the “Player”) acceptance and participation in the Netball Hong Kong, China Limited Netball Leagues, Events & Programs (the “Program”) and recognising the possibility of physical injury associated with netball. I hereby for the Player, waive and release, discharge and/or otherwise indemnify the Program and its associated personnel, including the owners of the fields and facilities utilised for the Program against any claim by or on behalf of the Player resulting from the Players participation in the Program that is now existing for damage or injury to the player, or to any person or property, resulting from the negligence or other acts of any employees or volunteers in connection with the players participation in the Program.

I further agree to indemnify and to hold the Program (including it associated personnel and subventing body LCSD – Leisure & Cultural Service Department) free and harmless from any loss, liability, damage, cost or expense which they may incur as a result of any injury and /or property damage that I or the Player may cause or sustain while participating in the Program. I understand that Medical Insurance coverage is the responsibility of the Player and that Netball Hong Kong, China Limited and LCSD does not provide medical insurance coverage for the Player.
